Prison/Ex-offender Case Manager

Provide case management services to individuals currently incarcerated and those recently released from the Philadelphia County Prison system. Provide consultation to other staff regarding prison issues on an as needed basis. Liaison with the staff of Prison Health Services, Defenders Association, court officers, probation and parole offices. Supervision of a Care Outreach Specialist.

Development Associate/Grant Writer

Grant writing, Donor Relationship management, and General Development Assist with the coordination and pursuit of grant opportunities to support the mission of ActionAIDS. This position will help to track grant opportunities, follow up and reporting, work with staff to coordinate the production and submission of these reports.
 

Behavioral Health Specialist Case Manager

Provide the full range of case management services to clients assigned to caseload. Carry a caseload of 35-40 clients. Provide back-up clinical coverage for direct service unit as part of direct service unit Case Management team.

Perinatal Case Manager

Provide the full range of case management services to assigned case load of HIV positive clients, including pregnant women, in clinical and/or office settings. Work with other identified program staff to decrease vertical transmission of HIV in consumers who receive services. Function as a part of the Direct Services team to ensure high quality services are provided to clients. 
This position is outstationed approximately two days a week at 2-3 perinatal clinics.
