Prison/Ex-offender Case Manager 

Qualifications: Minimum Bachelor’s degree, Master’s preferred, in social work or related field. Experience in human service delivery, case management, assessment and client advocacy. Understanding of the medical and psycho-social issues related to living with HIV/AIDS. Must be Bi-lingual Spanish/English.  Experience in corrections a plus. Ability to work with diverse populations. Working knowledge of local resources. Ability to work with various professionals, including medical, non-medical, and Para-professional staff in the case management and prison system. Eligible to enter the correctional facilities as a contract employee. Flexibility in work hours, including occasional evenings and weekends.

Basic Function: Provide case management services to individuals currently incarcerated and those recently released from the Philadelphia County Prison system. Provide consultation to other staff regarding prison issues on an as needed basis. Liaison with the staff of Prison Health Services, Defenders Association, court officers, probation and parole offices. Supervision of a Care Outreach Specialist.

Data Specialist and Quality Improvement Coordinator

Qualifications:

Bachelor’s degree in Social Work with strong interest and experience in research, management information systems, mathematics or other related field. Requires knowledge of statistics and ability to work with a variety of databases, create reports, and analyze data, familiarity with social service delivery systems a must. This position requires an ability to work well independently, be creative, flexible, manage schedule independently, complete tasks on time and effectively interact with colleagues. Successful candidate must be comfortable working in a multicultural environment.

Basic Function:

Produce statistical reports to support the mission of the organization and to provide required data to department directors, coordinators, external foundations, donors and funding entities.

Specific Responsibilities:

1. Create and produce monthly, quarterly and annual reports for Administration, Direct Service, Volunteer, Development, and Finance Departments.
• CareWare Reports
• Salesforce Reports
• MIP Reports
• Website data
2. Ability to identify data trends and communicate them effectively to management.
3. Review data with department Directors.
4. Produce, execute, and complete data analysis for annual Consumer Survey.
5. Responsible for internal Quality Improvement Audit.
6. Produce, execute and complete data analysis for the annual Board of Directors Survey.
7. Other duties as assigned.

Case Manager Specialist, Behavioral Health

Qualifications: Bachelor’s degree required in Social Work, or related field, Master’s preferred. Expertise in mental health/mental retardation, addiction, case management and client advocacy. Ability to work with diverse populations. Experience in guiding clients through critical and emergency situations. Sensitivity to cultural and economic differences. Knowledge of or ability to learn Philadelphia resources and services. Understanding of medical and psychosocial issues related to HIV disease and related conditions. Flexibility in work hours, including evenings and weekends.

Basic Function: Provide the full range of case management services to clients assigned to caseload. Carry a caseload of 35-40 clients. Provide back-up clinical coverage for direct service unit as part of direct service unit Case Management team.
