Medical Case Management
Medical Case Management is ActionAIDS’ core service and serves over 3,000 individuals and families each year out of over 40 locations. Our medical case managers are trained professionals and include specialists in the Deaf & Hard of Hearing, Drug and Alcohol Addiction, Families, Gay men, Immigrants, Mental Health, Nursing, Perinatal Care, Prison Systems, Transgender people, Women and Youth. Case managers speak English, Spanish, French, American Sign Language and Hebrew.
Establishing trust through one-on-one relationships, case managers and clients work together to coordinate care, navigate through complex governmental systems, and access medical, social, legal, and emergency services. Prisons today account for the largest concentration of people living HIV and AIDS. AtionAIDS provides case management, prevention, and outreach within the county prisons and keeps prisoners connected with medical and social services upon their release.
The program also offers Emergency Coverage 24 hours/7 days a week, Information and Referral, Direct Emergency Financial Assistance, and Clinical Supervision to smaller community providers. The excellence of ActionAIDS’ case management system is recognized both regionally and nationally.
Our services are divided into four primary programs:
