ActionAIDS' Youth Forward Program provides specialty Medical Case Management and Prevention services for adolescents and young adults. Services are tailored to meet the changing needs of this population which includes an educational and support approach to managing HIV disease.

Our Medical Case Managers work with youth to establish and develop a professional relationship to help remove barriers to accessing primary medical care. These services may include support groups, transportation, assistance with accessing medications and assistance and support for treatment adherence.
